added the option to add up to 3 courses in an application to form a study package;

packaged applications are visually marked by a stack icon on the left side of the main course, making it easy to see them;

only the main course in the packaged application will be displayed for simplicity however, hovering the stack icon will reveal all courses packaged in the application;

updated the invoice creation process to allow claiming commission for each course in the package;
